PARTICIPATION FORMS

All are due by August 1^st. However, your child will not be allowed to participate without their fully executed Medical Emergency Consent Form and Medical Questionairre and Physical Examination.

1. 1. Medical Emergency Consent Form - must be notarized.
   2. Medical Questionaire and Physical Examination – physical must have been performed after January 1, of current season  NO EXCEPTIONS!!!
   3. Birth Certificate:  Original bearing a raised seal - no copies!
   4. Report Card:  full academic year for current year. Exception Flag:  if child attended preschool the previous year.
   5. Pop Warner player contract.
